      1. No input focus field is provided.
      2. Tab order is not logical or topological. If the text field with the label "First Name" is selected and the user presses tab, the next text field selected is "Referred by", although the logical and topological selection would be "Last Name".
      3. The drop-down menu with the label "State" starts off with "BC" as its first item. "BC" is the postal abbreviation for British Columbia, a province of Canada, not a state. The menu item also does not follow alphabetical order as it is followed by menu item "AL".
      4. The drop-down menu with the label "State" includes menu items consisting of two-letter postal abbreviations for a particular state, with the exception of menu item "IND" (which one may assume stands for Indiana). The menu item's format is therefore inconsistent with other menu items.
      5. The drop-down menu with the label "State" does not include menu items for the states of Hawaii (HI), Nevada (NV), as well as the District of Columbia (DC). While the menu item "Other" is included, it would be more logical to include menu items "DC", "HI", and "NV" instead.
      6. The default values for drop down menus with the labels "State" and "Best Way To Contact Me" are not the same.
      7. The use of capitalization for labels is not consisent throughout the page. Some labels have all words capitalized (e.g. "Best Way To Contact Me"), while others do not (e.g. "Months left on plan").
      8. The tag for the second section of the Review, "Local/Long Distances/International Service" has an incorrect spelling for the word "Distances". It should be singular (i.e. "Distance") when used as part of a descriptive adjective for the word "Service".
      9. The drop-down menu with the label "Current Provider" in the "Cell Phone Service" section does not include the menu item "Other".
      10. The drop-down menu with the label "Current Provider" in the "Cell Phone Service" section is not in alphabetical order.
      11. The drop-down menu with the label "Months left on current contract" in the "Cell Phone Service" section does not have menu item "More", which would be logical provided that some individuals or companies could have more than 10 months on their current contract.
      12. The label that follows drop-down menu with the label "I last upgraded my phone(s) approximately:" in the "Cell Phone Service" section simply states "months", but should say "months ago".
      13. The drop-down menu with the label "Current Provider" in the "Local/Long Distances/International Service" section is not in alphabetical order.
      14. The drop-down menu with the label "Current Provider" in the "TV Service" section is not in alphabetical order.
      15. The drop-down menu with the label "Current Provider" in the "Home Security Service" section is not in alphabetical order.
      16. The drop-down menu with the label "Current Provider" in the "Gas and Electric Services" section is not in alphabetical order.
      17. The "I am opento using different provider" checkbox label in the "Gas and Electric Services" section is misspelled. It should read "I am open to using a different provider"

If the text field with the "Referred by:" label is left empty and the form is submitted with all other required fields completing, an error message shows up saying "Please enter the name of the person you were referred by." The "Referred by" label would therefore have to be preceded by an asterix.
      19. If the required fields are not completed and the form is submitted, the asterixes preceding the required field labels turn red with the exception of the label "Email".
      20. The text field label "Email" is preceding by an asterix, indicating that it is a required field. However, a user is able to submit the form without typing anything in that field.

          1. No matter what you are entering into the field "Email", whether it's valid email address or just random letters it still allow you to submit.
          2. Tab order in "Contact Information" is inconsistent in the case of "First Name" and "Last Name" (when you press ''tab" key, right after entering first name, it jumps straight to "Referred by" field).
          3. In "State" box selection some states abbreviation are wrong, some states are missing, "other" option available-which is unnecessary.
          4. In the form, part "2.Local/Long Distances/International Service", Tab order doesn't work on additional options like "TV", "Both", "No" (you can't move to those options with Tab order).
          5. In part "3" Tab order is not available for options like "Dial-up", "DSL", "Provider", "Phone", "Both".
          6. In part "4"- Tab order is not working on "Phone" and "Both".
          7. In part "7", there is misspelling in sentence "I am opento using different provider"-it should be "open to" instead of "opento".
          8. Error window pops up every time you push "Clear" button.
          9. Label "Referred by" needs asterix as it's one of required fields (system won't let you to submit form without entering this field).

            1. There is no default input focus.
            2. Tab order in Contact information is not logical: it skips several fields and goes from First name to Referred by. Should go from First name to Last name.
            3. Emali in Contact information is marked as a required field but in fact, the form can be submitted without it. - Functional bug.
            4. States in drop-down list are not in alphabetic order in Contact information.
            5. Section 1. Logically, it is more correct to use " I last upgraded my phone(s) approximately: ... months AGO" instead of "I last upgraded my phone(s) approximately: ... months."
            6. Section 1. "Months left on current contract" field: option "prepaid" should be removed from the drop-down list.
            7. Section 3. Fields "Bundled with" and "Type of service" should have check boxes instead of radio buttons, so user can easily uncheck the box if accidentally checked it.
            8. Section 7. "I am opento using different provider" field should be changed to "I am open to use different provider."
            9. Button "Clear" should be the same size as button "Submit."
            10. Button "Clear" does not work properly - it doesn't clear the fields. Also, an error window pops up. - Functional bug.

              1. "First Name" and "Last Name", "City", "Street Address" adn "Email" fields allow numbers and special characters
              2. "State" list contains wrong spelling for Indiana (IND instead of IN)
              3. "Zip code" field allows 11111 that is invalid
              4. Required field "*" by the "Phone" field is red while others are blue
              5. "Referred By" field allows special characters instead of letters
              6. General Data Section - TAB order issue. "Referred By" field is highlighted right after the "First Name" field
              7. Section 3 - TAB order issue. "Provider" field is not highlighted after the "Wireless Aircard" field

                1.The size of the buttons submit and clear are not simillar
                2.The first name field can include varios characters,and accept them.
                3.The last name field can include varios characters, and accept them.

                    5. After attempting to submit the empty form, and the warning message is shown under the main page header, “*” sign before “Email” text box wasn’t converted to red and remains blue.

                            1.After Submit , email is not sending.
                            2.First and Last Name accepted alphanumeric.
                            3. Email is accepting not email format.
                            4. Email is requirement but Submit with blank value.
                            5.City accepting alphanumeric.

                            7.Cell Phone Service section My monthly bill is approximately should be numeric but it is accepting non-numeric value.

                              1. Contact Information. Field “First Name” do not go to field “Last Name” with Tab Key, it goes to the field "Referred by".
                              2. Field "ZIP" is allowed letters.
