#1 2 Fields, which is supposed to be digits only accept letters as well (Fields are "My monthly bill is approximately", "ZIP").

#3 The label of "E-mail" field marked with "*" but actually is not considered as obligatory (essential).
#4 The is no default input focus on the form.
#5 Cell Phone Service: "Current Provider" list add "Other" value.
#6 Contact Information: Inconsistence in tab order (Reffered by " right after "First Name").
#7 "Clear" button does not clear the form. Instead when I push the "Clear" button I got confused and misleading warnig message "Error 17896 - Invalid data request. Please call technical support or restart your computer !!!"
#8 "Submit" and "Clear" buttons are not consistent in font size and dimensions.
#9 "State" listbox: remove "BC" item.
#10 "My monthly bill is approximately" (3 fields in the form) accepts not only figures but letters also if copy-pasted.
